Fairbanks Therapy Associates accepts both insurance and self-pay clients. Our office will file claims as a courtesy to you. Payment is due at the time of service. We accept cash and all major credit cards. We do not accept personal checks. As a courtesy we will bill your insurance company if all information has been provided by the client. We strongly recommend that new clients contact their insurance providers to determine their policies provider benefits. We also recommend that clients stay in touch with their insurance company, as insurance company payment rates and co-pay rates often change without notice. We will provide clients with monthly invoice for any unpaid balance.
Fees and Payments
Cancellation and No Show Policy
If a scheduled appointment must be cancelled or rescheduled, clients must provide 24 hour notice or clients will be charged a cancellation fee. Any emergency or unforeseeable situation will be considered on a case by case basis.
